When should we discontinue antiarrhythmic therapy for atrial fibrillation after abdominal surgery?

چکیده
Background The occurrence of peri-operative AF after abdominal surgery is a clinical condition burdened by several complications, especially in the elderly [1-3]. When AF is successfully converted to sinus rhythm, it is unlikely to recur, and nearly all of these patients are discharged from the hospital in sinus rhythm. It is not clear how soon these patients may discontinue antiarrhythmic therapy to avoid drugs side effects without risking recurrence of AF. The recurrence of AF needs different kind of treatment. Medical therapy includes various antiarrhythmic drugs to control heart rate and restore sinus rhythm and anticoagulation to reduce the tromboembolic risk [4]. Overactivation of sympathetic nervous system, related to surgery stress [5], could be reduced by the treatment with antiarrhythmic drugs, such as Beta Blockers, and may reduce the incidence of AF recurrence [6-10].

Catheter ablation and antiarrhythmic drug therapy as first- or second-line therapy in the management of atrial fibrillation: systematic review and meta-analysis.
BACKGROUND The optimal management of atrial fibrillation remains unclear. We performed a meta-analysis of randomized controlled trials to examine the safety and the efficacy of catheter ablation (CA) when compared with antiarrhythmic drug therapy both as first- and second-line therapy for the maintenance of sinus rhythm in atrial fibrillation. METHODS AND RESULTS Several databases were search...

متن کاملIntravenous Methylprednisolone, a Possible Cause of the Atrial Fibrillation
We are presenting a case illustrating atrial fibrillation (AF) following the use of methylprednisolone in a patient with pelvic and femur fracture. A 48- year- old man with no significant past medical history, was admitted to the emergency department after injury in a car accident. He suffered a multiple bone fracture with chief complaints of pain and shortness of breath. He was transfer...

AIMS Catheter ablation of the inferior vena cava-tricuspid annulus isthmus and continuation of antiarrhythmic drug therapy have been shown to be an effective hybrid therapy for atrial flutter which results from antiarrhythmic drug treatment of atrial fibrillation. The aim of this study was to determine the risk factors for recurrence of atrial fibrillation in patients undergoing hybrid therapy ...
متن کاملAntiarrhythmics After Ablation of Atrial Fibrillation (5A Study).
BACKGROUND Atrial arrhythmias are common early after atrial fibrillation (AF) ablation. We hypothesized that empirical antiarrhythmic drug (AAD) therapy for 6 weeks after AF ablation would reduce the occurrence of atrial arrhythmias. METHODS AND RESULTS We randomized consecutive patients with paroxysmal AF undergoing ablation to empirical antiarrhythmic therapy (AAD group) or no antiarrhythmi...
